Items 1 through 11--Identification

Item 12--Reporting cycle

Item 13--Operating injection pressures

• Report maximum recorded annulus pressure in the “Hydrocarbon or Gas” column for each reporting month.
• This information may not be applicable for a brine mining permit pursuant to Statewide Rule 81 if the cavern does not have a roof blanket

• Select code “4” for a well permitted under Statewide Rule 95 or 97 that is operating under temporary alternative monitoring status under section (o) of these rules.
• Select code “5” for a well permitted under Statewide Rule 95 or 97 that is operating under permanent alternative monitoring status under section (o) of these rules.
