RAILROAD COMMISSION ADOPTS NEW FORMS FOR CHAPTER 4 IMPLEMENTATION
On October 28th, 2025, the Railroad Commission (RRC) of Texas adopted new proposed forms:
- Form EP-1, Authorized Pit Registration/Change Form (Excel format)
- Form EP-2, Schedule B Authorized Pit Financial Security Worksheet
- Form EP-2A, Addendum to Form EP-2
- EP-5, Permit Application for Surface Management of Oil & Gas Waste (PDF and Excel Worksheet) (Replacing Form H-11 and Form R-9)
- Form EP-9, Generator Waste Profile
- Form EP-10, Waste Manifest
Use of Commission-approved forms is required and effective October 29, 2025.
The newly adopted forms will be available on the RRC Oil & Gas Forms web page https://www.rrc.texas.gov/oil-and-gas/oil-and-gas-forms/ and on the Environmental Permits and Support (EPS) Chapter 4 web page https://www.rrc.texas.gov/oil-and-gas/applications-and-permits/environmental-permit-types/all-about-chapter-4/.
